LifeSTREAM is Private Pay 

  • I only accept private pay. I can provide invoices you can submit to your insurance for out of network billing. I use the same billing codes as if it was an office setting. There is no special CPT code for equine work.
  • I do not offer a sliding scale rate due to the added expense of the equine facility and equine specialist.
  • My participation as credentialed group practice provider with Energy Matters is completely separate from LifeSTREAM Therapy LLC. No Equine Assisted Psychotherapy services are provided in that setting.

Therapy Session Rates

  • Individual session ages 13-64.  (50 minutes) — $180 (update 07/01/2023)
  • Intake session (50 minutes) — $200
  • NO additional costs for working with co-parents or families. Most providers do charge more. I do not work with couples in this setting.
  • I will provide invoice for therapist session time that can be submitted for out of network reimbursement.
  • Group rates: $75 for each 90 minute session. A separate, individual, Connect! screening session may be required for group participants. If coming with a referral from a current or recent therapist a check-in will be only requirement. This is to ensure a good fit and appropriateness of the client. In addition insurance will not reimburse you without a proper recent evaluation
  • Payment via card on file is required at time of service.

Good Faith Estimate of Expected Charges

This is for direct cash pay clients who are NOT using Out of Network provided “superbill” invoices.

You have the right to receive a “Good Faith Estimate”
explaining how much your medical care will cost
Under the law, health care providers need to give patients who don’t have
insurance or who are not using insurance an estimate of the bill for medical
items and services.

• You have the right to receive a Good Faith Estimate for the total
expected cost of any non-emergency items or services. This includes
related costs like medical tests, prescription drugs, equipment, and
hospital fees.
• Make sure your health care provider gives you a Good Faith Estimate
in writing at least 1 business day before your medical service or item.
You can also ask your health care provider, and any other provider you
choose, for a Good Faith Estimate before you schedule an item or
service.
• If you receive a bill that is at least $400 more than your Good Faith
Estimate, you can dispute the bill.
• Make sure to save a copy or picture of your Good Faith Estimate.


Rate increases may occur at the beginning of each year

This is regardless of length of time attending. Ongoing group participants will not have a change in fee.


How do I pay?

Payment is due at the time of service each session or in chunks for groups. A credit card must be provided before start of services and remain on file for billing. No cashiers checks please. Cash and all major credit cards are accepted.

Cancellation and No Show Policy

  • You must cancel at least 24 hours in advance of the session or you will be charged 100% of the fee.
  • Exceptions will only be made in the case of severe illness, death in the immediate family or weather that makes it unsafe to travel.

Health insurance varies greatly from plan-to-plan. Please call your provider to learn about the extent and limits of your coverage for mental health. Questions you should ask include:

  • What is my coverage for outpatient mental health?
  • What is the reimbursement rate for an out-of-network provider?
  • How many sessions are covered per calendar year?
  • What is my deductible and how much has been met?

Reimbursement for out of network benefits

If you would like to access your “out of network” benefits, I can provide you with an itemized Statement for Reimbursement form (also called a Superbill) for you to submit to your insurance company for possible reimbursement.

  • Our sessions use the same American Medical Association CPT (Current Procedural Terminology) codes as traditional therapy based on time (45-50, 60 minutes, etc.) and type (i.e intake, individual, family, group).
  • There is not a specialty code for equine assisted (or animal assisted) psychotherapy and no exclusion for this approach.

Other types of requests

  • Requests for new written documentation or forwarding existing documents such as assessments, treatment plans, progress notes, primary care provider summaries or termination summaries will be charged at $42/15 minutes. Fees are cash pay only.
  • Telephone consultations outside of scheduled session time will be charged $42/15 minutes. Fees are cash pay only.

Equine Assisted Psychotherapy Session Rates (on hold)

NOTE: The therapist fees are significantly lower than individual non EAP sessions, noted above, in order to make them more affordable.

We prefer — 90 minute — sessions to offer depth of interaction with the herd. These can be scheduled every two weeks. 60 minute sessions are possible as well.

  • Intake session (50 minutes at office or online not at EAP site–Licensed Therapist only) — $200 
  • Individuals/Couples/Co-parents/Families: $250/hour (60 minutes)
  • Fee includes Licensed Therapist @ $110/hour; Equine Specialist and one or more horses @ $110/hour; Facility Fee @ $30/hour
  • Billing and payment is separated by team member. I will provide invoice for therapist session time that can be submitted for OON reimbursement. Payment via card on file is required at time of service.
  • A separate payment for the certified equine specialist/facility is required immediately at time of service. Equine specialist will note payment options and provide a receipt. This will not indicate “therapy” in any way as it is not admissible for reimbursement.
  • Each additional portion of hour is rate @$250/hour. Note this additional time may not be reimbursed by insurance. They often do not allow more than 60 minutes in a day for outpatient. 
  • EAP Group rates: $75 for each 90 minute session. A separate, individual, Connect! screening session may be required for all group participants. If coming with a referral from a current or recent therapist a check-in will be only requirement. This is to ensure a good fit and appropriateness of the client. In addition insurance will not reimburse you without a proper recent evaluation.

  • Cancellation and No Show Policy

    • You must cancel at least 48 hours in advance of the session or you will be charged 50% of the fee. This covers the cost of the Equine Specialist/Horses and the facility. They block space that others could be paying for.
    • You must cancel at least 24 hours in advance of the session or you will be charged 100% of the fee.
    • Exceptions will only be made in the case of severe illness, death in the immediate family or weather that makes it unsafe to travel.
    • Note you can substitute a phone or HIPAA compliant Google Meet video session if needed. However if an EAP session the full fee is still required to compensate equine professional, horses and facility.
